Request a direct mailing of an Access Card by clicking the link below. Please do not submit more than one request per SNAP card. Multiple requests delay processing times. You may email dabux@hawaiifoodbasket.org to check on the status of your request.

The Access Card will be mailed to the address submitted on this form within two weeks of your request. This is a free service. You may use this form to request an Access Card if you do not currently have one or your Access Card was lost or stolen. All submitted information will only be used internally for program evaluation.

Sign Up In Person

Visit one of the locations below to sign-up for an Access Card. SNAP cardholders should bring their EBT card to prove they are an active SNAP recipient. You will receive your Access Card at the time of sign-up. In-person signs-ups are fast and easy, and takes only 5 minutes.
